智慧行動應用程式介入對腸造口病人居家自我照護能力及生活品質之成效
Effects of a Home Care Mobile App on the Outcomes of Discharged Patients with a Stoma: A Randomized Controlled Trial
摘要
背景：腸造口照護行動應用程式可以提升病人的自我照護能力和生活品質，然而國內行動應用程式對腸造口病人生活品質、自我照護及造口相關併發症等成效的相關研究缺乏。目的：探討腸造口照護行動應用程式對腸造口病人出院生活品質、自我照護及造口相關併發症之成效。方法：採隨機對照實驗性研究，診斷為大腸癌需要進行暫時性腸造口手術病人，共有實驗組47人，控制組47人，實驗組接受常規出院衛教及智慧行動應用程式，對照組接受常規出院衛教，兩組在出院日及出院第4個月以結構性問卷收集資料。結果：研究結果顯示無論實驗組或控制組，在出院日及出院4個月的自我照顧能力，隨著時間的推移兩組均有改善，實驗組在生活品質領域中與家人朋友關係範疇、社交範疇及心理範疇優於對照組；在出院4個月實驗組造口合併症發生率較低（x^2=16.04, p=.01）具顯著差異。結論：行動應用程式可以提供病人居家中使用，提升病人在生活品質中與家人朋友關係、社交及心理領域的關係，降低造口合併症發生。本研究成果可以提供暫時性造口病人在出院後面對調適適應。

Background: Ostomy care app can improve patients' self-care ability and quality of life, but research on the efficacy of ostomy care app on ostomates' quality of life, self-care and stoma-related complications is still insufficient. Purpose: This study aims to investigate the effect of ostomy care app on quality of life, self-care ability and stoma-related complications of ostomates after discharge. Methods: Randomized control and experimental designs were adopted. Patients who were diagnosed with colorectal cancer and required temporary colostomy were enrolled. There were 47 patients in the experimental group and 47 in the control group. The experimental group received routine discharge education and mobile smart application, while the control group received only routine discharge education. Data were collected from both groups by structured questionnaire before discharge and in the fourth month after discharge. Result: The results of the experiment showed that the self-care ability in ahead of discharge and after 4 months of discharge improved in both groups over time. The experimental group had better outcome than the control group in quality of life, which included relation to family and friends, social contact and psychological compromise. Incidence of stoma complications in 4 months after discharge was lower in the experimental group with statistical significance (x^2=16.04, p=.01). Conclusion: Ostomy care app can be used at home to improve the patients' quality of life with family and friends, social and psychological relationships, and reduce the incidence of ostomy complications as well. The result shows that ostomy care app can provide temporary ostomates benefits and empower them to cope with their situation after discharge.
