如何利用聽力測試的程序變化篩檢假性聽損者

Identification of Pseudohypacusis Using Various Test Procedures

摘要


根據文獻顯示,施測於合作的病人身上,純音或語音聽力檢查,無論使用何種測試程序,兩者聽閾值的差距應該在8-10分貝之間。另外我們都知道純音、語音聽閾值的不一致性亦可做為判斷假性聽損的指標。 本研究比較兩組對聽力檢查程序熟悉與不熟悉的正常聽力者,用口頭及文字解釋偽裝聽障的原因,並鼓勵他們假裝聽力喪失;用三種不同的聽力檢查方法(1)純音聽力上升檢查法(2)純音聽力下降檢查法(3)語音閾值上升檢查法,試圖找出最大純音、語音聽閾值的差距。 本研究有三個目的(一)用不同的測試程序找出最大純音或語音聽閾值的差異(二)瞭解是否經過練習熟悉聽力檢查的測試程序,就可以得到一致的純音或語音聽閾值(三)建立一個有效的假性聽損篩檢方法。 結果顯示,用改變測試程序所產生的閾值差距,在兩組假裝聽力喪失者之間並無顯著不同。證明可以利用每個人判斷音量大小的響度認知偏差,針對疑似有假性聽損動機者做聽力檢查時,語音測試使用上升檢查法,純音測試則用下降檢查法,就可以找出純音、語音聽閾值的最大不一致性;熟悉或不熟悉聽力測試程序都無法改變這個事實。

The objectives of this study are: first, to determine the pure-tone average-spondee threshold (PTA-SRT) agreement between two psudohypacusis test procedures; second, to investigate whether familiarity with test procedure can improve response consistency; third, to propose a test battery for the explanation of threshold differences between two test procedures. Twenty unsophisticated participants and ten clinical audiologists with normal hearing were asked to simulate hearing impairment. Three conventional audiologic measurements were carried out in two test procedures. We concluded that differences between procedures may result from loudness bias. pure-tone average-spondee threshold (PTA-SRT), we recommend audiologists to employ an ascending procedure in measuring SRT and to employ a descending procedure in measuring PTA.
