糖尿病患者性功能障礙與糖尿病控制指標之相關探討－以南台灣一所醫學中心為例
Correlation between Sexual Dysfunction and Diabetes Control in Patients with Diabetes: A Cross-Sectional Study in Medical Center in Southern Taiwan
摘要
性功能障礙是糖尿病患者常見的問題，性健康也是臨床糖尿病照護的一部分，若忽略未處理，可能造成患者其心理上的低自尊，並影響其生活品質。本研究目的在於探討糖尿病患者性功能障礙的盛行率及糖尿病控制指標與性功能障礙之相關性。本研究採橫斷式研究，以方便取樣於台南某醫學中心蒐集糖尿病患者資料，採結構式問卷進行調查，包括人口學屬性、男性國際勃起功能指標問卷量表、女性性功能指標量表問卷量表，再透過病歷瀏覽方式，蒐集美國糖尿病衛教學會建議十項糖尿病控制指標，並進行相關性分析。本研究總共有224位糖尿病患者參與，分析124位（55.4%）自訴有性行為之糖尿病患者，由其性功能障礙比例得知，男性糖尿病患者勃起功能障礙盛行率為55.8%（43/77）；女性糖尿病患者性功能障礙為63.8%（30/47），糖尿病患者合併有性功能障礙者相較於性功能正常者，其e-GFR末期（19.2% vs. 9.2%）及神經病變（17.8% vs. 9.8%）比例均高出兩倍，而性功能障礙與抽菸有顯著相關（χ^2 = .625, p ＜ .044）。研究結果發現，糖尿病患者之性功能障礙不論性別均具有高盛行率，腎功能異常、神經病變在糖尿病患者中仍有過高的比例。另外，抽菸與性功能障礙有其相關性，在未來應加強糖尿病患者中已有腎功能異常、神經病變者及抽菸者進一步諮詢性健康相關問題，以期即早處理並轉介。

Sexual dysfunction is a common problem among patients with diabetes. Sexual health should be considered as part of clinical diabetes care. If the sexual health of the patients is ignored, it might lead to their low self-esteem; furthermore, affecting their quality of life. This study aims to probe into the prevalence of sexual dysfunction caused by diabetes and the correlation between diabetes control and sexual dysfunction. By using a cross-sectional method, the diabetes patients' data were collected and investigated in a medical center of Tainan through convenience sampling. A structural questionnaire includes demographic variables, IIEF and FSFI. Medical records were also reviewed to collect data of the ten diabetes control indexes recommended by American Diabetes Association for correlation analysis. This study had in total 224 participating patients with diabetes. From the analysis of the percentage of sexual dysfunction 124 (55.4%) patients with diabetes who reported to have sexual dysfunction. It is found that the prevalence of erectile dysfunction among the male patients with diabetes was 55.8% (43/77) while the female patients with diabetes who also suffered from sexual dysfunction is 63.8% (30/47). The patients with diabetes and complicated sexual dysfunction were almost twice as likely as their counterparts with normal sexual function to develop into late-stage e-GFR (19.2% vs. 9.2%) and neuropathy (17.8% vs. 9.8%). Moreover, there is a significant correlation between sexual dysfunction and smoking (χ^2 = .625, p ＜ .044). Patients with diabetes who also suffer from sexual dysfunction, regardless of gender, have high prevalence rates of renal dysfunction and neuropathy, both of which are excessively high among patients with diabetes. There is a correlation between their smoking and sexual dysfunction. It is therefore important to provide further consultation on health issues for patients with diabetes who smoke and already have renal dysfunction and neuropathy to obtain early handling and referral.
