醫療價值創新與梅迪奇效應之相關性研究

The Relationship between the Innovation of Medical Value and the Medici Effect

摘要


目的:「梅迪奇效應」(Medici Effect)意指不同領域、學科或文化的想法交會時,創造出大量的、突破性的創新想法。而醫療產業卻在「專科」和「次專科」制度下分工愈細,此恐不利於醫療整合與價值創新。本研究旨在瞭解醫療價值創新的內涵,以及跨領域醫學人才的知識整合造成突破性醫療價值創新的情形,並進一步以梅迪奇效應建構醫療價值創新的階段性模式。 方法:由於醫療價值創新的定義未明,又醫療產業缺乏跨領域知識整合之相關研究,本研究以質性方法切入,針對國外1個、國內6個國家級與醫學中心級醫療研究團隊,共11位研究員進行相關議題之深度訪談,訪談方式除海外受訪者為電話訪問之外,其餘皆在受訪者研究室進行面對面訪談。訪談結果利用紮根理論以及內容分析法進行語幹萃取,類目及研究假說之建構。 結果:研究結果提出醫療價值創新的定義與影響創新的因素,並發展三大假說與子假說。其中,醫療價值創新可以「臨床效果」、「成本效益」及「學術貢獻」等三大構面來衡量;團隊與個人背景的多元性可對於醫療價值突破性創新績效發揮梅迪奇效應而產生正向影響;以及跨領域人才醫學創新歷程可分為摸索期、適應期與快速轉換期等,梅迪奇效應則在適應期開始發酵,並在快速成長期發揮最佳效果。 結論:多元背景所產生的梅迪奇效應有助於醫療價值突破性創新。本研究結果期待可成為醫院管理者與醫學教育工作者在規劃臨床以及醫學創新研究團隊時參考的方向。

Objectives: The ”Medici effect” refers to remarkable, breakthrough innovations and insights at the intersection of different ideas, disciplines, and cultures. In the healthcare industry, the development of ”specialties” and ”sub-specialties” has resulted in a division of labor and a subsequent decline in medical integration and value innovation. This study explored the concept of ”medical value innovation”, as well as how multi-disciplinary medical research teams facilitate the integration of knowledge and enhance breakthroughs in the innovation of medical value. A staged model of medical value innovation from the viewpoint of the Medici effect was also constructed. Methods: The definition of ”innovation of medical value” was unclear and there were no related studies on multidisciplinary knowledge integration in the healthcare industry; therefore, a qualitative approach was used to conduct in-depth interviews with seven medical research teams, including one from a national research institute and five from four medical centers in Taiwan, as well as one from a university in the United States. A total of eleven researchers were interviewed. Grounded theory and content analysis were utilized to analyze the data, select themes, and construct categories and research hypotheses. Results: A definition for ”innovation of medical value” and the factors that influence it were determined and three major hypotheses and their sub-hypotheses were constructed from the results. Medical value innovation can be measured by ”clinical effect,” ”cost effectiveness” and ”academic contribution”. In addition, the multidisciplinary backgrounds of team members and team leaders can produce the Medici effect and positively influence breakthrough innovations. The staged model of medical value innovation from the viewpoint of the Medici effect includes an exploration stage, an adaptation stage and a rapid conversion stage. The Medici effect begins to form at the adaptation stage, and has its greatest effect at the rapid conversion stage. Conclusions: Multi-disciplinary backgrounds result in the Medici effect and facilitate the innovation of medical value. This study can provide a reference for hospital managers and medical educators when planning and organizing a medical innovation research team.
