目的:將原本紙本簽署之同意書導入無紙化電子簽署,完成電子病歷無紙化最後一哩路。方法:以團隊協同合作並建置同意書電子簽署系統,範本模式結合醫療系統自動識別病患資訊產生同意書,避免人工填寫錯誤和疏漏,電子系統進行簽署和管理,簽署記錄在系統中保留簽署過程的數據及時序,具不可否認性、完整性及不可偽造等資訊安全機制。結果:2020年1月成立專案小組,2021年3月全面導入同意書電子簽署,計6860類醫療項目實施電子簽署。結論:電子製作及儲存同意書,具環保節能、節省人力、儲存空間及時間,提升服務品質、民眾滿意度、降低成本等效益,利於推動HIMSS EMRAM國際驗證,更獲得APSAA亞太暨台灣永續行動獎,將醫療品質與病人安全照護達到國際世界級水準。
Objectives: The purpose of this project is to digitize the paper-based consent forms and implement electronic signatures, thus completing the final mile of paperless electronic medical records. Methods: The project involves collaborative teamwork to build an electronic consent form signing system. It combines a template-based approach with the healthcare system to automatically identify patient information and generate electronic consent forms. This eliminates errors and omissions that may occur during manual form filling. The electronic system handles the signing and management process, recording the data and timeline of the signing process within the system. It incorporates information security mechanisms such as non-repudiation, integrity, and anti-tampering measures to ensure the authenticity and integrity of the signed documents. Results: The project team was established in January 2020, and the electronic consent form signing was fully implemented across the entire hospital in March 2021. A total of 6,860 different types of medical items were subjected to electronic signatures. Conclusions: Making and storing the consent form electronically is environmentally friendly and energy-saving, saves manpower, storage space and time, improves service quality, public satisfaction, reduces costs and other benefits, and is conducive to promoting the HIMSS EMRAM international certification, also honored by APSAA silver prize, bringing medical quality and patient safety care to the international level World class.
