以所羅門四組設計進行運用多媒體執行團體衛教之成效探討

Effectiveness of Multimedia with Solomon Four-Group Design on Group Education

摘要


背景:外科手術病患病情穩定後即返家自我照護,其中傷口照護是必備的能力。因此醫護人員必須於病患出院前,提供病患或家屬傷口照護相關資訊以促進傷口癒合。目的:探討運用多媒體執行團體衛教,對手術後病患照護知識、技能之影響,並進一步了解護理成本之差異。方法:採所羅門四組設計,在五個外科病房先抽出4個單位,再抽兩單位採新的方案,兩個採用原本的教學方案,共收得實驗組及控制組共121人。實驗組患者接受團體衛教(專任護理師運用多媒體執行),控制組則接受傳統性衛教(責任護士1對1床邊口頭衛教)。以結構式問卷進行資料收集,再以SPSS/PC套裝軟體進行統計分析。結果:發現在知識方面,兩組之成效相當,技能方面團體衛教優於傳統衛教,而衛教所發費的成本則顯示團體衛教措施較傳統衛教節省。結論/實務應用:整體而言接受團體衛教效果優於傳統衛教,此結果能作為未來相關衛教計劃擬定之參考。

Background: Wound-care skills are essential when a patient is in a stable condition after surgery in order to allow them to return home for self-care. Health care professionals must teach wound-care skills to patients or their families before discharge to help wound-healing, and explain the symptoms and signs of infection.Purpose: The purpose of this study was to investigate the influence of group teaching on the wound-care knowledge and skills of surgical patients and the possibility of reducing teaching costs.Method: The research was based on the Solomon four-group design to study four surgical wards in a medical teaching hospital. Two wards used the new program, and the others used the original teaching program. In total, 121 surgical patients were selected according to purposive sampling and divided into the experimental group and the control group. The experimental-group patients received group education from in-charge nurses using multimedia materials, and the control group received traditional one-to-one oral guidance. Patients' performance was measured using a structured questionnaire before and after education, and data were collected and analyzed by SPSS/a PC software package.Results: The experimental group and the control group wound-care knowledge among the four groups did not differ significantly; however, the level of skill was significantly different (p<0.05), and the costs of care and health education were reduced.Conclusion/Practical Implications: To summarize the above, group education was better than traditional care in terms of quality and efficiency. It is hoped that this study will form a reference for other health education programs.
