Serotonin syndrome (SS) presents as a combination of clinical manifestations that are associated with adverse drug effects. Diagnosis of this syndrome relies on past history, clinical symptoms, and careful evaluation by clinicians. Here, we report a 37-year-old woman with a history of major depression who was receiving trazodone treatment. The patient experienced symptoms such as painful bilateral leg swelling, involuntary tremors, and agitation for one day, and SS was subsequently diagnosed. Complications including rhabdomyolysis and acute compartment syndrome also developed. SS was controlled after cyproheptadine therapy, and rhabdomyolysis and acute compartment syndrome resolved after surgical fasciotomy and intensive care. The patient was discharged after 35 days of hospitalization.
