運用跨理論模式於一位血液透析病人水分控制之護理經驗

Nursing Experience of Applying Transtheoretical Model in Water Control of A Hemodialysis Patient

摘要


本篇運用跨理論模式結合實證於一位血液透析病人水分控制之護理經驗,照護期間2020年11月3日至2020年12月31日,筆者藉由整體性評估、觀察、個別訪談及查閱電子病歷紀錄等收集資料,以跨理論模式為基礎,評估及根據病人所經歷的動機前期、動機期與準備期等三個行為改變階段,依其個別性設定目標,運用行為改變策略,以及實證文獻搜尋結果設計生津止渴之護理措施,讓病人實際感受執行水分控制對身體的好處,使能自我察覺,促使行為的改變,進而將限水習慣融入日常生活中。在繁忙的護理實務工作下,藉由理論引導整合實證輔以措施介入,盼此未來運用於臨床,協助病人導正錯誤認知增進自我學習的動機,促使達到個別性、專業性與整體性之護理照護。

This article uses transtheoretical model combined with evidence -based on the nursing experience of water control in a hemodialysis patient. The nursing period is from November 3, 2020 to December 31, 2020. The authors collect data through holistic assessment, observation, individual interviews, and electronic medical records. The assessment is based on transtheoretical model, and the three stages on the patient's experience of precontemplation, contemplation and preparation. We set goals on their individuality, use behavior change strategies, and empirical literature search results to design nursing strategies for promoting body fluid and quenching thirst. Let patients actually feel the benefits and integrate water restriction habits into daily life. Under the busy nursing practice, we integrate empirical evidence with theoretical guidance and intervention with measures. We hope that this will be applied to the clinic in the future to help patients correct their misunderstandings and enhance their motivation for self-learning, so as to promote individual, professional and holistic nursing care.
