一位行達文西甲狀腺腫瘤切除之手術全期護理經驗
A Full-stage Nursing Experience in Surgical Surgery for Da Vinci Thyroid Tumor Resection
摘要
本篇是探討一位甲狀腺腫瘤患者接受達文西機器手臂之手術全期護理經驗。護理期間自2019年3月5日至2019年3月15日。筆者以手術室護理人員的角色，在手術前、中、後期透過觀察、傾聽、會談、參閱病歷紀錄及運用身體、情緒、智能、社會、靈性五大層面等評估方式收集資料。了解到個案在手術前期：焦慮／與將面對使用達文西機械手臂、缺乏手術訊息及擔心手術結果有關。手術中期：潛在危險性手術情境傷害／與使用達文西機械手臂輔助採截石臥位、時間冗長及電燒器止血相關。手術後期：急性疼痛／手術傷口疼痛。在護理過程中，筆者藉由手術前訪視，與個案及家屬建立良好互動關係，並透過術前的訪視、手術中安全照護、手術後面臨問題衛教及出院後的電訪，給予個案採用完整性的手術全期照護，有助於提升個案自我照顧能力，達到最佳健康狀況，並且能儘快恢復以往之生活。

This article discusses the experience of a patient with a thyroid tumor who underwent full surgical care with the da Vinci machine arm. The period of care was from March 5, 2019 to March 15, 2019. In the role of the operating room nursing staff, the author collected data during the pre-, mid-, and post-operative periods through observation, listening, interviewing, reviewing medical records, and using the five major dimensions of assessment: physical, emotional, intellectual, social, and spiritual. Pre-operative: anxiety/association with the use of the da Vinci arm, lack of information about the surgery, and concerns about the outcome of the surgery. Mid-surgery: Potentially dangerous surgical situation injuries/associated with the use of the da Vinci robotic arm to assist in the lithotomy position, prolonged time, and electrocautery to stop bleeding. Post-operative: acute pain/pain in the surgical wound. During the care process, the author establishes a good interaction with the patient and family through preoperative visits, intraoperative safety care, postoperative problem health education, and post-discharge telephone visits to provide the patient with complete surgical care, which helps to enhance the patient's self-care ability, achieve the best health status, and return to life as soon as possible.
