本文是運用自我管理於蜂窩性組織炎合併初患糖尿病個案,個案因傷口造成生理上不適感,加上初次罹患糖尿病缺乏照護知識及技能,及對工作、經濟負擔造成心理上壓力,表現出不安及不確定感。自2021年6月2日至6月18日,藉由Gordon十一項健康功能型評估為工具,確立個案護理問題為現存性感染、增進健康自我管理的準備度及焦慮。照護過程運用自我管理概念及跨團隊照護,改變不良飲食及生活習慣,提供個別性照護,藉由疾病衛教及心理支持,給予正向鼓勵及引導表達感受,提供糖尿病相關照護知識及技能、清創傷口及心理問題等護理措施,讓傷口及血糖控制穩定,個案透過生活型態改變,提昇個案自我健康管理能力,希望藉此照護經驗分享,提供類似個案之照護參考。
This article aims to apply the principles of self management to help the patient with newly-found diabetes mellitus who suffered from cellulitis. The patient expressed much anxiety and worry, which may be the results of physical discomfort caused by wounds, lack of nursing skills, and stress from job and financial burden. Since June 2 to June 18, 2021, we assessed the patient's status by Gordon's eleven health functional patterns, and found that the patient's main problems of care were the existing infectious status, worry for health self-management, and anxiety. We used the concepts of self-management and cross-team care to make a change in unhealthy diet, living habits, individualized care. It also creates positive encouragement and guidance for the patient by expressing his feelings appropriately. Through education and psychological support, the patient learned better nursing skills for wound care, which could in turn stabilize the wound and benefit control of blood sugar. Via change of lifestyle, the patient could improve their ability of health self-management. I hope to share the nursing experience and provide reference for similar cases.
