協助一位肺癌病人參與臨床試驗之護理經驗

癌症病人為了尋求治療與生存的機會，有越來越多的人加入臨床試驗，本文為探討一位肺癌病人參與臨床試驗的照護經驗。照護期間為2014年4月1日至6月10日，以羅氏適應模式為評估架構，發現個案有不確定感、皮膚完整性受損及無望感的護理問題。護理過程中主動關懷與個案建立良好的護病關係，並運用同理心、傾聽等技巧給予適當的支持，重整希望感受。對於初次參與試驗的不了解，偕同醫師舉辦家庭會議降低不確定感，並針對藥物副作用給予正確的護理指導。臨床試驗在護理界是一個較新的領域，臨床護理師對於臨床試驗的概念是陌生的，期望藉此報告分享，讓臨床護理師在照顧參與臨床試驗的病人能提供更多的幫助。

In search for effective treatment and a chance of survival, the number of cancer patients enrolling in clinical trials have been on the rise. The article explored a nursing experience of a lung cancer patient enrolling in clinical trial. The nursing period lasted from April 1st to June 10th, 2014. With Roy's adaptation model as assessment framework, the following nursing issues were identified: uncertainty, skin integrity, and hopelessness. A positive nurse-patient relationship was established during the nursing process. Empathy and listening were used to provide adequate support to the patient to regain a sense of hope. To reduce uncertainty induced by first-time enrollment in clinical trials, a family meeting was held with the doctors, in which proper nursing guidance was provided on the side effects of clinical trial medications. Clinical trial is a relatively new discipline in nursing. Clinical nurses are often unfamiliar to the concept of clinical trial. We hope by sharing this nursing experience provide assistance to clinical nurses when attending to patients enrolling in clinical trials.
