提升護理師執行正顎手術護理之完整性
A Project of Improving Completion Rate of Orthognathic Surgery Nursing Care
摘要
正顎手術能解決顏面功能性障礙與外觀，完整的護理指導與預後有密切關係，本專案旨在提升護理師執行正顎手術護理照護之認知與完整性。臨床上，因護理指導不一致造成病人抱怨，故以問卷調查及實際觀察法得知護理師執行正顎手術護理完整性僅64.1％、認知正確率僅64％，原因為：（1）.護理指導內容不一致，缺乏定期教育與標準指引、（2）.照護流程不完整。藉專案執行：（1）.正顎手術標準作業流程與稽核制度、（2）.製作臨床交班指引單、（3）.舉辦在職教育與床邊擬真教學、（4）.跨團隊合作衛教影片等。護理師執行正顎護理完整性提升至96.3％，手術認知正確率提升至97.3％，顯著提升總體改善護理照護品質與滿意度。

Comprehensive nursing care is closely related to the prognosis of surgery. This project aims to enhance the awareness and performance integrity of nursing care for orthognathic patients. To examine the causes of clinical complaints arising from inconsistent nursing guidance questionnaires and actual observations, revealed that the degree of completeness of nursing care in performing orthognathic surgery was only 64.1%, and the degree of cognitive accuracy was only 64%. Our findings indicated that the following three factors contribute to the problem: 1) Inconsistent nursing instruction content, 2) Lack of regular education and guidelines, and 3) Incomplete nursing processes. For the purpose of this project, we implemented the following items: 1) To set up a standard operating procedure and regular audit program for orthognathic surgery, 2) To develop clinical handover guidelines, 3) To arrange on-the-job training at the bedside, and 4) To prepare cross-team cooperation health education videos. As a result, the completeness of orthognathic care performed by nurses increased to 96.3% and the correct rate of patient understanding of surgery increased to 97.3%. This outcome indicated that a targeted intervention can successfully improve completion rate of orthognathic surgery nursing care and improve patient understanding of medical procedure.
