一位肺炎病人因跌倒導致肋骨骨折併血胸之急診照護經驗
An Emergency Nursing Experience in Caring for a Pneumonia Patient with Rib Fractures and Hemothorax Following a Fall

本文描述一位因感染肺炎前往醫院就醫過程中跌倒致左肋骨骨折合併血胸之急診照護經驗。照護期間為2021年12月17日15時至12月19日16時30分筆者透過直接照護、身體評估、會談及出院電訪等方式收集資料，運用Gordon十一項健康功能性型態為架構，確立健康問題有急性疼痛、呼吸道清除功能失效、照顧者角色緊張。照護過程中感受到個案因肋骨骨折及胸管留置產生疼痛問題，因疼痛而無法有效咳痰，期間運用傾聽、同理關懷建立治療性人際關係，教導個案和家屬胸廓固定技巧、胸管照護及拍痰方式，改善疼痛及強化清除痰液技巧，協助家屬共同參與疾病照護改善照顧者緊張問題。建議單位舉辦在職教育課程，提升同仁對肺炎鏈球菌疫苗知識。

Emergency care for a pneumonia patient with rib fractures and a hemothorax is described in this article. The care period is from 15:00 on December 17 to 16:30 on December 19, 2021. The patient fell while seeking treatment for pneumonia, resulting in a left rib fracture and a hemothorax. During care, data was collected through direct observation, physical assessments, interviews, and discharge phone calls, using Gordon's health functional models. The patient's health issues included acute pain, respiratory clearance dysfunction, and caregiver stress. Recommended that units organize in-service education courses to enhance colleagues' knowledge of the pneumococcal vaccine.
