頭頸癌病人於放射線治療期間之營養狀態分析

Analysis of Nutritional Status of Head and Neck Cancer Patients During Radiotherapy

摘要


頭頸癌病人放射治療期間,可能會發生黏膜發炎、吞嚥困難等副作用,造成病人的進食量與體重下降、身體組成改變,嚴重時可能影響治療和預後。研究目的為分析頭頸癌病人放射治療期間各階段營養指數變化。共收集2021年10月到2023年6月間,頭頸癌病人30-35次門診放射療程期間,完整介入4次營養諮詢衛教,且於放射線治療第一週與最後一週,各完成一次身體組成分析儀檢測之病人,整理其飲食的熱量及蛋白質攝取的情形,與體重及身體組成之瘦體組織的變化。結果顯示:病人於放療期間熱量攝取量逐漸減少:第一週1798.08 ± 429.61 kcal/day、最後一週1599.56 ± 489.61 kcal/day達顯著差異(p = .001),另蛋白質攝取量第一週為81.124 ± 28.11 g/day、最後一週78.42 ± 27.56 g/day(p = .45);體重變化從66.95 ± 13.58公斤,下降到64.55 ± 12.83公斤(p = .16);體組成之瘦體組織指數從49.73 ± 11.22公斤,下降到47.57 ± 10.47公斤(p = .12)。進一步分析第一周和最後一周瘦體組織增加或減少兩組發現,當熱量攝取達每公斤體重25大卡以上、蛋白質攝取達每公斤體重1.3克以上,體重及瘦體組織才不會減少,應納入日常衛教諮詢注意飲食攝取調整建議之重點。

During radiotherapy for head and neck cancer patients, side effects such as mucosal inflammation and dysphagia may occur, causing the patient's food intake and weight to decrease, and body composition to change. In severe cases, it may affect treatment and prognosis. The purpose of the research is to analyze the changes in nutritional index at each stage of head and neck cancer patients during radiotherapy; a total of 4 complete nutritional consultations and health education sessions were collected during 30-35 radiotherapy courses for head and neck cancer patients in outpatient clinics from October 2021 to June 2023, and in the first and last weeks of radiotherapy, patients who completed a body composition analyzer test were sorted out to assess their dietary calorie and protein intake, as well as the changes in body weight and body composition of lean body tissue. The results showed that the patient's caloric intake gradually decreased during radiotherapy: 1798.08 ± 429.61 kcal/day in the first week, 1599.56 ± 89.61 kcal/day in the last week, a significant difference (p = .001), and protein intake in the first week was 81.124 ± 8.11 g/day, and finally 78.42 ± 27.56 g/day in one week (p = .45); the weight change dropped from 66.95 ± 13.58 kg to 64.55 ± 12.83 kg (p = .16); the lean body tissue index of body composition dropped from 49.73 ± 11.22 kg to 47.57 ± 10.47 kg (p = .12). Further analysis of the two groups with an increase or decrease in lean body tissue in the first week and the last week found that when the caloric intake reaches more than 25 Kcal per kilogram of body weight and the protein intake reaches more than 1.3 grams per kilogram of body weight, the body weight and lean body tissue will not decrease. This should be included as the key points of dietary intake recommendations in daily health education consultation.
