Varicocele is an ongoing disease which is a dilatation of the internal spermatic vein and the pampiniform venous plexus. It occurs in approximately 10-25% of all males. Among these cases, 90% occur in the left hemiscrotum,10% involved bilaterally and rare in right hemiscrotum. Usually, dull, aching, testicular atrophy and decreased fertility are presented with varicocele. Varicoceles with severe symptoms may interfere with flight security. Besides, the association with malignancy in right side varicoceles should not be neglected. We reviewed domestic and foreign military physical standards and waiver guidelines about varicocele to provide insights for the aeromedical and waiver consideration of pilots with varicocele.
