空軍飛行員頸椎病變手術後恢復飛行文獻探討

The return to flight of Air Force pilots after surgery for cervical spondylosis - a narrative review

摘要


本文目的在瞭解戰鬥機飛行員罹患頸椎病變經過手術治療後恢復飛行的情況,或接受頸椎前位椎間盤切除及融合手術或頸椎人工椎間盤置換術對頸椎椎間盤突出的飛行員恢復飛行的影響。作者從飛行安全的角度來檢視文獻對於戰鬥機飛行員接受頸椎前位椎間盤切除及融合手術或是頸椎人工椎間盤置換手術的預後。儘管統計數據上顯示頸椎損傷經過治療後恢復飛行的比例很高,但也有飛行員在復飛之後因為症狀復發或者心理因素等狀況,而改變職業生涯的方向甚至退役。另外亦發現儘管在飛行安全及醫療考量上對於頸椎病變能否繼續飛行已有相關規定,但對於戰鬥機飛行員而言,在珍貴人力資源的角度上,還是可能從缺點免計精神使整體的需求得以滿足。最後,作者也發現不少適合本國空軍、軍事研究人員、軍醫有研究潛力的課題。

To investigate the differences in the return to flight of fighter pilots with cervical spine pathology following surgical treatment such as anterior cervical discectomy and fusion (ACDF) or cervical artificial disc replacement (ADR), we reviewed published literature focusing on the impact of these procedures on flight safety. While the available statistics indicate a high rate of successful return to flight after treatment for cervical spine injuries, it is important to note that some pilots have chosen to change their career path or even retire from military service due to symptom recurrence or psychological factors. Despite the existence of the waiver guide and medical assessments regarding the ability to continue flying, there are still opportunities for fighter pilots to adapt and to meet their overall needs within the context of these regulations. To conclude, our review has highlighted several areas that warrant further investigation by the Air Force, military researchers and physicians. These include exploring the long-term outcomes and potential challenges associated with the return to flight of fighter pilots who have undergone ACDF or ADR. Identifying strategies to address recurrent symptoms and psychological factors that may affect pilots' ability to sustain their flying careers also worth investigation. We can improve our understanding of the complex interplay between surgical interventions, flight safety and the overall well-being of fighter pilots by addressing these research gaps.
