以往一般顱內動脈瘤是用傳統的血管攝影來檢查,本研究是利用迴轉式數位減贅血管攝影(Rotational digital subtraction angiography, DSA),及經過工作站重組成為三度空間立體影像(3D Reconstruction),用來評估動脈瘤頸部的形狀方向及周圍血管的關聯性。在收集15位患有顱內動脈瘤病患,全部接受迴轉式數位減贅血管攝影與三度空間立體重組影像。3D重組血管影像後,可隨意旋轉出各種不同角度,是2D沒有的,特別是在動脈瘤的患者,均能在手術或血管治療前被精確的診斷出,尤其血管影像在動脈瘤栓塞治療時,第一條可分離式線圈的選擇測量是很重要的。在沒有重覆輻射暴露及過多造影劑使用下,能幫助我們更瞭解顱內複雜血管的解剖,增加動脈瘤治療前之診斷評估的精確度。
Traditionally, the imaging diagnosis of cerebrovascular diseases was using conventional angiography or digital subtraction angiography(DSA).The purpose of this study was to compare the capability between the 2-dimension(2D) and 3-dimension(3D) reconstruction images of the rotational digital subtraction angiography (DSA) for the diagnosis of the intrtacranial aneurysms. The imaging evaluation includes the visibility of the aneurysm, the location, direction, and size of the aneurysm neck, and the relationship between the aneurysm and the surrounding vessels. 15 patients having intracranial aneurysms were enrolled. All of them underwent rotational DSA, with 2D and 3D reconstruction. This is especially crucial in the preoperative evaluation of the intracranial aneurysms, for which either surgical clipping or transarterial coil embolization was planed. The sizes of the aneurysm and its neck are very important for choosing the sizes of the detachable coils. Moreover, the 3D reconstruction can help delineating the relationship between the aneurym and the surrounding arteries, without adding to the radiation exposure and contrast medium administration. It improves the confidence in the diagnosisand treatment of intracranial aneurysms.
