摘要


高解晰度超音波HRUS已經成為臨床檢查肌肉關節疾病一種非常有用和普遍的工具,然而在急診處的使用上卻沒有那麼普遍,軟組織的異物殘留可以透過X光以及臨床的觸診檢查出來,但是對於X光以及臨床觸診無法檢查出的異物殘留,急診或門診醫師將無法於第一時間檢查出來,根據文獻報導,有38%的異物殘留無法於第一時間檢查出來,因此在美國急診病患控告急診科醫師的醫療糾紛排名第二的就是軟組織異物殘留。軟組織異物殘留的延誤診斷,會造成類似過敏、發炎、膿瘍、肉芽腫及廔管等併發症的發生。這項研究的目的是如何偵測在軟組織內非放射性顯像異物的殘留,我們使用高解晰度的超音波,從2000年10月至2002年10月,23位軟組織受傷並懷疑有異物殘留的病患,在臨床觸診及X光皆顯示無異常的狀況下安排高解晰度的超音波檢查,我們總共偵測出39例病灶,手術取出36件異物,有3例為陽性。高解晰度超音波的運用,將可提高非放射線異物殘留的偵測,除了可降低此疾病的併發症外也可減少因此類病症而產生的醫療糾紛。

The purpose of this study is to demonstrate the application of HRUS (High Resolution Ultrasound) for detection of non-radiopaque foreign bodies retained in soft tissue. From Oct.2000 to Oct. 2002, 23 patients with soft tissue injuries and suspected of retained foreign bodies were studied by HRUS. There were totally 39 foreign bodies not detected by both palpation and radiographs but recorded by HRUS. Surgical explorations was done and removal of 36 foreign bodies. Three false positive observations of HRUS were noted. HRUS is a good modality to detect the radiolucent foreign bodies and it might decrease the possibility of lawsuit in emergency room.
