目的:質子治療作業中,因一台迴旋加速器提供質子輪流給四台治療機,造成病患擺位程序完成後,需長時間等候治療射束的轉換,而需長時間維持姿勢而容易產生變化與不適,同時影響治療。材料及方法:治療室病患安排,遵守Request時間點再進入治療室,進行擺位與治療程序,收集前後四台治療室各25天,紀錄並分析病患等候治療射束時間。結果與討論:每天等候射束平均總時間沒做與有做時間差分別是58.71±6.85分鐘與46.70±4.16分鐘,差異約12分鐘,p=0.024有明顯的差異。每個等候治療射束平均時間在沒做與有做時間差分別是2.84±3.25分鐘與2.62±3.12分鐘;p=0.019有明顯的差異。同性質治療室1、2沒做與有做時間差分別是2.86±3.27分鐘、2.92±3.3分鐘;p=0.672無明顯的差異。治療室3、4沒做與有做時間差分別是2.82±3.23分鐘、2.38±2.96分鐘,p=0.001有明顯的差異。各治療室的每個等候治療射束平均時間在沒做與有做時間差分別是G1是2.69±3.12分鐘、3.03±3.61分鐘,p=0.104;G2是3.02±3.4分鐘、2.81±2.94分鐘,p=0.287都沒有明顯差異;G3是2.82±2.87分鐘、2.39±3.12分鐘,p=0.014;G4是2.82±3.54分鐘、2.38±2.78分鐘,p=0.020都有明顯的差異。結論:利用時間差可以減少病患等候治療射束時間,並降低病患的不舒適度及增加治療準確性。
Purpose: In proton therapy, patients are required to lie flat and wait on the treatment bed for the conversion of the beam before treatment. The displacement resulted from maintaining the posture with long time might lose the advantages of proton therapy. This study attempts to use the time difference to reduce wait time and provide the stability of the treatment posture. Material/methods: We recorded patients' waiting time from rooms 1 and 2 with the same wobbling system (Sumitomo Proton therapy System) and rooms 3 and 4 with the same pencil beam system. There were 25 working days for each time-dividing and non-time-dividing. For non-time-dividing, number of patients in room 1, 2, 3, and 4 are 262, 289, 219, and 231 and number of irradiation fields in room 1, 2, 3, and 4 are 528,584, 523, and 533, respectively. For time-dividing, number of patients in room 1, 2, 3, and 4 are 222, 210, 260, and 262 and number of irradiation fields in room 1, 2, 3, and 4 are 466,464, 682, and 621, respectively. The waiting time difference of time-dividing or not between the treatment rooms was analyzed by using the independent T-test of SPSS Vision18 software. Results: With all four treatment rooms simultaneous operation, the average waiting time of everyday without and with time-dividing are 58.71±6.85 and 46.70±4.16 minutes(p = 0.024), the difference was about 12 minutes, and the average waiting time of all proton beam without and with time-dividing are 2.84±3.25 and 2.62±3.12 minutes (p=0.019).Both have a significant difference. With the same characteristics treatment room 1 and 2, the average waiting time without and with time-dividing of proton beam are 2.86±3.27 and 2.92±3.3 minutes (p=0.672),In rooms 3 and 4, the average waiting time without and with time-dividing of proton beam are 2.82±3.23 and 2.38±2.96 minutes (p=0.001),In rooms 1, 2, 3, and 4, the average waiting time without and with time-dividing of proton beam are 2.69±3.12 and 3.03±3.61 minutes (p=0.104), 3.02±3.4 and 2.81±2.94 minutes (p=0.287), 2.82±2.87、2.39±3.12 minutes (p=0.014), and 2.82±3.54 and 2.38±2.78 minutes (p=0.020), respectively. The percentages of waiting time more than 60 minutes without and with time-dividing are 35.2% and 16.7%, respectively. Conclusion: In proton therapy, the use time-dividing could reduce the patient's waiting time, decrease the discomfort of patients, minimize the random error of treatment, and improve the stability.
