An 80 years old patient with rectal cancer was admitted for surgical intervention. During the operation, lack of transverse and descending colons were found accidentally. The ascending colon went up to the hepatic flexure then turned to the retro-peritoneum space below the ascending colon and became sigmoid colon from the terminal ileum area. In our experience such kind of colon rotation has never been found before, even in the previous documents. Besides, it is an atypical type, if regarded in the group of malrotation. We report this case.
