Congenital Retroperitoneal Transverse and Descending Colons- An Atypical Type of Midgut Malrotation in Adult

先天性後腹腔位橫、降結腸－非典型中腸旋轉不良

摘要

此為一術中發現之病例。病人為一八十歲男性，因直腸癌入院接受手術。手術時發現其大腸走向異常，升結腸走至肝曲處即下降至後腹腔，再從盲腸附近進入腹腔成為乙狀結腸及直腸。以中腸旋轉不良來說，大部分均於嬰兒或兒童時期發現，因早期即有症狀。成年病例仍屬少見，且以第二階段之完全不旋轉(nonrotation)型較多。本例乃一無症狀成人，且其若歸於中腸旋轉不良，亦屬非典型之第三階段腹膜化不良型之變異，提出供大家參考。

An 80 years old patient with rectal cancer was admitted for surgical intervention. During the operation, lack of transverse and descending colons were found accidentally. The ascending colon went up to the hepatic flexure then turned to the retro-peritoneum space below the ascending colon and became sigmoid colon from the terminal ileum area.
In our experience such kind of colon rotation has never been found before, even in the previous documents. Besides, it is an atypical type, if regarded in the group of malrotation. We report this case.
