Purpose. To assess the use of intradermal methylene blue in cases of pruritus ani irresponsive to standard primary, dermatological, and colorectal care.Methods. Eighteen patients excluded for other etiologies that may cause pruritus ani, and proved irresponsive to standard care received 5 mL 0.1% methylene blue diluted with 5 mL 1% xylocaine and epinephrine into the perianal skin intradermally.Results. After one injection of the above solution, 16 (89%) patients were symptom-free. The other two patients became symptom-free after a repeat injection one month later. Only one patient developed a complication (parathesia of perianal skin). There was no recurrence at six months. At 12 months, one recurrence was noted. The early response rate was 89% and became 100% with a repeat injection.Conclusion. Intradermal methylene blue injection is a safe, simple, fast and efficient management of intractable idiopathic pruritus ani.
