本圈之目的是為了降低本院日間留院個案管理師接案工作疏漏率。收集92年7月至9月期間院個案共30,以病歷審核、個案問卷方式,查核接案工作疏漏情形,發現接案工作疏漏率高達30%。依真因驗證結果,發現疏漏原因有:工作繁雜,容易疏漏;工作人員認知差異;缺乏工作手冊;工作人員執行不夠徹底;病人精神狀況不佳等。針對這些疏漏原因擬定下列對策加以改善:簡化工作流程、製作接案工作手冊、備忘錄及指引單、訂定審核制度、給新進人員教育訓練等。在所有策略宣導、執行、審核後,至92年程月中測時,疏漏率即已下降至15%;至93年3月15日,接案工作疏漏率更下降至3%,目標達成率高達180%、進步率90%。顯示此品管圈活動之實施確實發揮顯著功效。
A complex medical field such as that which deals with psychiatric disorders can only meet the expanding problems of quality management by multidisciplinary cooperation. The quality control circle (QCC) is the central element of the quality control process and is essential to realize further advances in quality management. The purpose of this study was to apply the quality control circle in the Taipei City Psychiatric Center, Taiwan. The goal of this QCC was to reduce the missing rate of tasks in before-hospitalization arrangements, which was initially found to be 30%. Data from 30 patients admitted to daycare for psychiatric disorders from July to September 2003 were collected and analyzed. After the QCC activity, the missing rate of tasks in the before-hospitalization arrangements was reduced to 3%. The target achievement rate was 180%. The results show that the quality control circle is an excellent tool for quality improvement and management.(Full text in Chinese)
