透過攜帶型自動血壓儀測定(Ambulatory Blood Pressure Monitoring, ABPM),可以得知24小時血壓的變動與多種循環指標變化的訊息。運用ABPM之研究發現,高血壓病患若屬於夜間血壓非下降者(non-dipper),其心臟血管意外之危險性較夜間血壓下降者(dipper)為高。ABPM也可發現晨間急劇血壓升高(morning surge)之存在,而且morning surge與早上腦中風等血管病變之相關性也受到相當的重視。透過ABPM的監測,可以注意到治療高血壓時是否有夜間降壓過度或morning surge控制不良等問題。ABPM之測定,除了可以推測24小時血壓的變動與心、腦等標的器官傷害的關連之外,對臨床診斷、治療高血壓以及對標的器官傷害的進一步了解應具有相當助益。
With the aid of ambulatory blood pressure monitoring (ABPM), we may identify the variations in blood pressure over a 24-hour period and gather information on various circulatory parameters. The application of ABPM has revealed the higher risk of cardiovascular mortality in hypertensive patients of non-dipper than dipper group. ABPM may detect the existence of morning surge correlating with cerebrovascular accident in the morning. Treatment with excessively antihypertensive effect in the night or poor control of morning surge may also be found with ABPM. With the aid of ABPM, it may be useful in speculating the correlation between variations in blood pressure over 24-hours and target organ (such as heart or brain) damage, in the clinical diagnosis and treatment of hypertension and in giving further understanding of target organ damage.
