P4醫學精神在居家醫療整合照護的實踐－北市聯醫的經驗
The Realization of P4 Medicine Spirit in Integrated Home Medical Care: the Experience of Taipei City Hospital
摘要
台灣目前正面臨高齡化及少子化的挑戰，老年及失能人口不斷增加，加上社會家庭結構改變，導致家庭照顧人力的缺乏，更可能造成失能者因行動不便而無法外出就醫治療的困境。北市聯醫在105年初開始投入中央健保署「全民健康保險居家醫療整合照護計畫」，提供因失能或疾病特性致外出就醫不便患者，整合性居家醫療的照護服務。本文將探討北市聯醫在建置其服務模式的過程中，如何應用國際最新的P4醫學的精神，以提升失能或弱勢患者醫療服務之可近性，提供可靠性、連續性、周全性及協調性，並落實在地化的服務。期能透過分析北市聯醫在流程、案例、困境上的經驗，提供台灣醫界在實務上與政府政策推動時參考。

Taiwan is currently facing the challenge of an aging population and low birth rate, and an increasing size of elderly and the disabled, as well as a structural change of society and families. As a result, the elderly and disabled may be immobilized and find it more and more difficult to go out for medical treatments due to a lack of family caregivers. Since the beginning of 2016, the Taipei City Hospital joined the “Integrate Home Medical Cared Project＂ approved by the National Health Insurance Administration, Ministry of Health and Welfare, to serve people with disabilities, especially those with low-income. This article discusses the development process of the service model designed by the Taipei City Hospital, including applying the spirit of the latest international well-known P4 medicine, to enhance accessibility of medical care service for the disabled or disadvantaged patients, and to provide localized health care service with reliability, continuity, comprehensibility and coordination. Through the analysis of the flow procedures, case examples and predicament of the Taipei City Hospital, we contribute our experience to other medical institutions in Taiwan and as a reference for promoting government policy.
