2019年新冠狀病毒感染(Coronavirus-19 disease; COVID-19)嚴峻疫情未歇,牙科診療作業直接接觸病人口腔口沫,交叉感染風險極高,而COVID-19病人接受治療藥物若產生口腔部位的藥物不良反應,亦可能會尋求牙醫診療。COVID-19治療藥物相關之口腔不良反應雖尚不顯見,但其中抗病毒藥物、干擾素及類固醇等曾有口腔不良反應相關之研究報告。蛋白酶抑制劑可能有腮腺脂肪瘤、味覺障礙、口乾症和口周感覺異常等口面部相關不良反應。干擾素治療可能導致唾液流速降低,衍生相關口腔健康問題。COVID-19治療期間使用皮質類固醇,可能導致口腔念珠菌感染等。COVID-19治療藥物對於口腔健康之影響及相關性有待臨床研究驗證,臨床診療時亦須予追蹤評估監測,必要時採取適當的防治措施,確保病人口腔健康及照護品質。
The coronavirus disease 2019 (COVID-19) infection is severe, and the pandemic has yet to stop. Dental diagnosis and treatments involve contacting the patient's saliva, which is associated with an extremely high risk of cross-infection, and Covid-19 infected patients with oral adverse drug reactions may seek dental treatments. Although oral adverse reactions related to COVID-19 pharmacotherapy are not noticeable, some related research reports have described the adverse drug reactions caused by anti-viral drugs, interferons, or steroids. The protease inhibitors may cause parotid lipomatosis, taste disturbances, xerostomia, and perioral paresthesia; interferon treatment may cause a decrease in saliva flow rate leading to related oral health problems; the use of corticosteroids or potent antibiotics during COVID-19 treatment may cause oral candida infections. The impact of COVID-19 treatment drugs on oral health and their relevance is yet to be verified by further clinical research, and their treatments should be followed up and monitored, and appropriate preventive measures should be taken to ensure the patient's oral health and quality of care.
