提升出院病人傷口自我照護正確率之專案
Improving the Wound Care Correction Rate of Out Patients
摘要
本單位為一般醫學外科病房，80%病人身上都有傷口，出院後常因傷口照護不佳導致返診次數增加，故引發改善動機。專案期間為2010年6月至2011年6月，審核出院病人傷口自我照護平均正確率為72.4%，其影響因素為單位無傷口換藥步驟單張及衛教審核標準、護理人員未交班說明已衛教病患傷口換藥程度，以及無早期介入衞教換藥措施，病人或主要照顧者無法在短暫一次衛教後正確執行換藥，經修訂單位傷口衛教標準作業、提升護理人員傷口照護專業能力、制定單位適用之傷口換藥衛教進度交班單等改善措施，參考其他醫院傷口照護指標做為標竿學習，出院病人傷口自我照護正確率由72.4%提升至91.3%，達到專案設定目標。

In the general medical surgical ward, there were 80% patients with wounds when they allowed backing to home. However, these patients came back to out patient department frequently for dealing with the wound problem by themselves. Due to figure out the wound care problem at home, we investigated and realized correction rate of patients practiced would care was 72.4%, and reasons were insufficient educational documents, hand-over shift incompletely on the would care, and too late to educate patients or their care givers. Therefore, we revised the educational documents, set up the flow-sheet and standard of hand-over shift and educational timing. Ultimately, the patients wound care correction rate improved to 91.3%, improved this problem and achieved the project goal.
