在急診室最常遇見因車禍或運動傷害而造成膝關節損傷的患者,一般由臨床科醫生判定是否需要執行X 光Merchant view 以評估髕骨脫位或骨折。此次研究共蒐集120 位因膝關節損傷而於急診執行X 光Merchant view 的患者,其中60 例採傳統Merchant 輔具照相,另外60 例採自製Merchant 輔具攝影。本研究使用李克特量表(Likertscale)三點計分法,分別針對患者配合度與放射師操作方便性作滿意度調查;並嘗試比較兩種攝影法在影像品質和患者受檢時間的差異。研究結果顯示,使用自製Merchant 輔具進行X 光Merchant view 攝影,患者配合度的滿意度從23.3%提升至60.0%,放射師操作方便性的滿意度從8.3%提升至58.3%;兩種攝影法在影像品質上並無顯著差異;而患者受檢時間更由原本的3.6 分降低至1.1 分。本研究證明,利用自製Merchant 輔具攝影,可以在維持相同影像品質的前提下,有效提高患者的配合度、增進放射師操作的方便性、並大幅縮短患者受檢時間。
Injury of the knee is the most commonly encountered trauma event in the emergency room. A Merchant view of knee joint X-ray is often required to evaluate patellar dislocation or fracture. In this study, a patient positioning device was used as an assistive device for the modified Merchant technique. Total 120 patients who admitted in the emergency room with suspicious patella injury were enrolled. Traditional Merchant technique was adopted for 60 patients, and modified Merchant technique was adopted for resting 60 patients. The patient comfort, the accessibility for the technologist, the image quality for the radiologist, and the examination time were compared between the two groups. The research revealed significant improvements in both patient comfort and the accessibility for the technologist in the group underwent the modified Merchant technique. The image quality showed no significant difference between the two groups. Moreover, the examination time decreased from 3.6 minutes to 1.1 minutes. In conclusion, the patient positioning device for the modified Merchant technique can provide significant improvements in the patient comfort, the accessibility, and the examination time, as comparing with traditional technique.
