懷孕婦女進行腹部X光攝影的器官劑量評估之研究

摘要


本研究旨在評估懷孕婦女進行腹部(KUB)X光攝影時,照野範圍內及附近器官的輻射劑量,特別關注性腺、乳腺、眼球水晶體、甲狀腺,以及胎兒的心臟。研究使用臨床設定的診斷型X光機,搭配CIRS ATOM® phantom模擬懷孕婦女進行X光檢查,並利用nanoDots光激發光劑量計進行測量及模擬。結果顯示,在照野範圍內,性腺受到1.66 mSv的劑量,而乳腺組織在照野邊緣僅為0.025 mSv。眼球水晶體和甲狀腺的劑量幾乎為零。胎兒的總劑量範圍為1.86~9.14 mSv。研究強調,雖然胚胎在懷孕初期暴露於極高劑量(超過1 Gy)可能致命,但應謹慎以診斷劑量為唯一中止妊娠的依據。總體而言,本研究提供了懷孕婦女進行KUB X光攝影的相關器官劑量資訊,以協助醫學決策和風險管理之參考。

The study aims to assess the radiation doses to organs within and near the irradiation field during KUB X-ray imaging in pregnant women, with a specific focus on the gonads, breasts, crystalline lenses of the eyes, thyroid, and the fetal heart. The research utilized a diagnostic X-ray machine under clinical settings, employing the CIRS ATOM® phantom to simulate pregnancy and nanoDots optically stimulated luminescence dosimeters for measurements. Results indicated that within the irradiation field, the gonads received a dose of 1.66 mSv, while breast tissue at the field edge received only 0.025 mSv. Doses to the crystalline lenses of the eyes and thyroid were nearly negligible. The fetal dose ranged from 1.86 to 9.14 mSv. The study emphasizes that while exposure to extremely high doses (exceeding 1 Gy) during the early stages of pregnancy may be fatal to the embryo, caution is warranted in considering diagnostic doses as the sole basis for pregnancy termination. Overall, this research provides pertinent organ dose information for pregnant women undergoing KUB X-ray imaging to assist in medical decision-making and risk management.
