The first permanent molars are emerging in the oral cavity without precursory teeth and often mistaken as primary teeth. The progression of occlusal caries would be rapid due to the micromorphology of the pit and fissure. (There will be high success rate if operator can choose the suitable case of deep occlusal caries in immature permanent molar for pulp capping.) The pulp capping would be a successful procedure to treat certain deep caries of young permanent molars and preserve their vitality. This case presents a 7-year-and-7-month-old boy diagnosed with deep occlusal caries in both mandible permanent first molars with opened apical foramens. By the direct and indirect pulp capping in either tooth, we successfully kept the pulp vitality, made the root formation complete and avoid the possibility of complex of endodontic treatment. Through proper diagnosis and case selection, pulp capping provides a tentative treatment for immature permanent teeth with deep caries in the mixed dentition.
