In the endodontic treatment of the primary teeth, stainless steel files were used traditionally for root canal debridement and shaping. Recently, Ni-Ti rotary instruments were also applied in the treatment. However, cautions must be taken to avoid unexpected fracture of the Ni-Ti files. This report presented a six-year-one month old boy with a broken Ni-Ti instrument beyond the distal canal of the lower right primary first molar concomitant with root resorption. We extracted the tooth and removed the broken file. The possible causes of separation of Ni-Ti rotary instrument and the techniques to remove broken files were discussed.
