根叉穿孔是乳牙中不樂見的狀況,它可能會導致牙周破壞及乳牙早期脫落而產生後續問題。三氧礦化物(mineral trioxide aggregate, MTA)已廣泛使用於恆牙牙根穿孔和牙根叉穿孔的修復,但在乳牙很少被使用。本篇病例報告是一位4歲7個月大的男童經轉診至本院,希望能治療左下第二乳臼齒的持續膿腫;經臨床及牙科放射線學檢查後,發現左下第二乳臼齒牙根叉穿孔,因此安排後續治療。左下第二乳臼齒經過根管治療及以三氧礦化物(MTA)覆蓋牙根叉穿孔後,牙齒並無明顯不舒服,且根叉病變減輕,因此進行不鏽鋼牙套之復形。經過15個月的追蹤,左下第二乳臼齒使用功能良好且無相關不適,根尖放射線檢查也發現牙根叉齒槽骨有修復癒合的現象。因此,三氧礦化物(MTA)亦可用為乳臼齒根叉穿孔修復的合適材料之一。
Furcation perforation is an undesirable situation in primary teeth, and it may result in periodontal destruction and early shedding. Mineral trioxide aggregate (MTA) has been used in repairing of root perforations and furcation perforations in permanent teeth, but rarely used in primary teeth. A 4 years 7 months-old boy was referred to our hospital for the treatment of persistent buccal abscess over his primary mandibular left secondary molar. The periapical x-ray and clinical examinations showed furcation perforation on the tooth. After management with root canal treatment, MTA was placed over the perforation site and the tooth was then restored with stainless steel crown. The tooth was asymptomatic and the radiolucent furcal lesion subsided following the treatment. After 15-month follow-up, the tooth remained asymptomatic and the furcal crestal bone healed. Therefore, MTA may be suggested as a suitable material for the repair of furcation perforation in primary molar teeth.
