在齲齒處理上的生物薄膜壓力與益生菌

Biofilm Stress and Probiotics in Caries Management

摘要


我們人體中很多地方都有許多細菌組成的生物薄膜(biofilm)存在,我們口腔內的生物薄膜若是因細菌失衡產生生物薄膜壓力,就有可能會形成牙菌斑、齲齒以及牙齦的感染,所以我們希望能藉由益生菌來導正細菌失衡的情形,以達到控制齲齒的效果。在回顧了許多研究文獻後可以推論其實益生菌是有潛力對抗生物薄膜壓力的,但因為本身的許多限制,仍然需要更多的臨床驗證。而母親是小孩齲齒細菌傳染的高度危險因子,藉由媽媽吃益生菌來達到預防齲齒細菌傳染的效果仍無法證實。細菌治療法也只是降低生物薄膜壓力來預防齲齒的方法之一,但只靠這方法就想達到控制齲齒的目的是不切實際的。

Biofilm was composed of many bacteria. It exists on many parts in human body. If there were any imbalance in bacteria, biofilm stress will be induced. This stress will cause dental plaque formation, dental caries, and gingival infection. Controlling bacte-rial imbalance by probiotics maybe beneficial to the aim of caries control. There are many evidences in the literature that probiotics have the potential to reverse bacterial imbalance in biofilm. The effect of maternal ingestion of probiotics to prevent the verti-cal transmission of caries bacteria from mother to the child is still to be verified. Pre-vention of caries merely by bacterial therapy to lower biofilm stress is not practical.
