Resorption of primary teeth is a normal physiologic process, except that occurs prematurely. A case of idiopathic root resorption identified in a girl aged 5 years and 8 months with no history of systemic disorders is presented. The patient visited pediatric dental department of National Taiwan University Hospital for management of multiple asymptomatic caries in the primary dentition. Radiographic examination revealed early root resorption of the upper right second primary molar, bilateral upper first primary molars, and all lower primary molars, with no obvious radiolucent apical lesion. Carious lesions of lower primary molars were not close to pulp both on clinical and radiographic findings. Bilateral upper permanent first molars were taurodont teeth. A histological examination of extracted upper left first primary molar revealed increases in blood vessels, lymphoplasmacytic cells, and histiocytes in remained pulp tissues . Some bacterial colonies resemble actinomyces were also identified. However, no obvious hyperemia or heavy inflammation was noted in histological examination of extracted upper right first primary molar. Further studies are needed to assess the relationship between periodontopathic bacterial species, taurodontism, occlusal force and idiopathic root resorption. Due to the premature loss of bilateral upper first primary molars, a Nance appliance was applied to prevent upper molars from rotating or moving forward. Conditions of root resorption after treatment need long-term observation.
乳牙牙根吸收是正常的生理現象,但不包括此現象過早發生。本案例為一位五歲八個月女童的自發性牙根吸收,家長否認兒童有任何系統性疾病的病史。該童來到台灣大學附設醫院的兒童牙科,主訴為處理口內多顆無症狀齲齒。放射學檢查結果顯示右上第二乳臼齒、雙 側上顎第一乳臼齒和所有下顎的乳臼齒有牙根吸收,沒有明顯的放射線可透性病灶。在臨床和放射學檢查結果上,下顎乳臼齒的齲齒皆未侵犯至牙髓,雙側上顎恆牙第一大臼齒是牛狀齒。對拔除之左上第一乳臼齒進行組織學檢查,顯示牙髓中有血管增生、淋巴漿細胞和組織球增加,還觀察到類似放射線菌的細菌菌落。然而,右上第一乳臼齒的組織學檢查中,未觀察到明顯的充血或嚴重發炎。需要進一步的研究來評估牙周致病菌、牛狀齒、咬合力和自發性根吸收之間的關係。 由於雙側上顎第一乳臼齒的過早喪失,因此使用南斯氏裝置以防止上顎臼齒旋轉或向前移動。經過治療後牙根吸收的狀況需要長期觀察。
