A 9-year-old girl came to our OPD with her parents for oral examinations. Apart from multiple caries in her mouth, we found no sign of upper right second permanent premolar with carious right deciduous maxillary second molar and periapical abcess. After extraction, the space was closed spontaneously by permanent first molar mesial tipping movement. However, we found a tooth germ between the upper right first permanent premolar and upper right first permanent molar when the girl was 11-year-old, and space regaining treatment started. Hence, we should take delayed odontogenesis into consideration when making diagnosis of congenital missing, especially for the developing patient.
