本文在描述ㄧ位初次罹患急性A型主動脈剝離病患,因接受緊急手術,術後面臨生理不適、擔心病情進展及身處陌生的環境,引發筆者想協助度過術後相關生、心理健康護理問題。主要照護期間為2010年10月3日至10月7日。經由觀察、會談、身體評估及直接照護等技巧,針對個案生理、心理、社會及靈性進行整體性評估並收集資料,確立個案有低效性呼吸型態、急性疼痛及焦慮等健康問題。在護理過程中,筆者運用良好的護病關係,提供符合個案之護理措施,協助處理呼吸困難及疼痛之控制。藉由熟悉加護病房特殊的環境和治療以及家屬的支持,進而減輕焦慮程度。期望藉此護理經驗,提供護理人員在未來照顧此類病患之參考。
This article describes an experience of postoperative intensive nursing care of an acute aortic dissection type A patient who had physiological and psychological problems after the emergency operation. From the period 3rd to 7th October 2010, we helped him to deal with the physiological and psychological problems. We evaluated the patient's condition in different aspect, such as physiology, psychology, spirituality and social interactions, by observations, history taking, health assessments and direct nursing cares. The result of nursing evaluation in this case showed that he has problems of ineffective breathing pattern, acute pain, and anxiety. Our using processes provide individual care and trusting relationship which help him ameliorating the dyspnea and pain control. In addition, the patient's anxiety levels were able to be reduced through improved familiarity with the unique environment of the intensive care unit as well as good support from his family. Therefore, this article can provide useful information for other nurse staff who care patient with similar situation in the future.
