肛門瘻管手術後復發中醫治療病例報告
Treatment of Anal Fistula after Operation with Recurrence with Traditional Chinese Medicine: A Case Report
摘要
一位五十九歲男性患者，在四年前出現發燒及肛門疼痛的症狀，經診斷為肛門旁癰腫後，行手術切開引流。兩個月後又出現發燒和肛門疼痛的問題，經電腦斷層檢查診斷為馬蹄形肛門瘻管，再次進行手術挂線療法。半年後又因為持續肛門疼痛、出膿和再次發燒而接受手術治療。在接受中醫治療前半年，發燒合併肛門疼痛和出膿又開始，這次不願意手術，只接受手法排膿，但之後仍反覆疼痛和滲膿。中醫治療初期使用清熱解毒兼以扶正排膿法，外用紫雲膏活血生肌，讓症狀得到緩解。治療中因為長途旅行後症狀加重，改用清熱解毒利濕法治療，外用紫雲膏，清除濕毒，膿毒排盡。最後再應用滋補心腎、安神定志法，改善病患長期失眠、頭痛等症狀，讓病情獲得完全改善。

A 59 year old man had fever and anal pain 4 years ago. He accepted incision and drainage operation while being diagnosed para-anal abscess. After 2 months, he got fever and anal pain again. He accepted the second operation (seton procedure) because CT scan showed out the existence of horseshoe type anal fistula. For continuous anal pain and pus discharge, he took operation again at 6 months since the first episode. Half year before accepting traditional Chinese medicine, he had fever, anal pain and discharge again. But this time he did not want surgery. He accepted manual procedure to expel the pus. Because recurrent anal pain and discharge, he accepted the therapy of clearing heat and resolving toxin combined with supplementing qi and the blood, and external use of zi yun gao. He got relief of the symptoms. During the course of therapy, he had increment of symptoms after a long trip. He had improvement of symptoms after changing to the therapy of clearing heat, resolving toxin and dispelling dampness, and the external use of zi yun gao. At the end, we use the therapy of nourishing yin of heart and kidney and quieting the spirit to eliminate his insomnia and headache.
