照護一位肝癌復發併呼吸衰竭面對疼痛之護理經驗
Nursing Experience of a Recurrent HCC Patient with Respiratory Failure and Pain
摘要
本篇為照護一位肝癌復發併呼吸衰竭面對疼痛之護理經驗，於2014/07/02至2014/07/14期間，筆者依據Gorden十一項功能性健康型態進行評估，歸納出主要護理問題有：一、呼吸道清除功能失效/與無效性咳嗽有關；二、急性疼痛/與癌症疼痛有關；三、焦慮/害怕死亡、疾病不確定感有關。護理期間，在呼吸道清除功能失效方面，透過適切呼吸器照護及胸腔物理扣擊，並衛教個案有效深呼吸咳嗽技巧，使個案得以成功脫離呼吸器。在疼痛方面，透過適切疼痛控制措施及衛教放鬆方式，個案可正確執行疼痛控制技巧來緩解疼痛。在焦慮方面，藉由主動關懷及傾聽感受，並提供醫療照護上的相關知識衛教，有效改善焦慮問題。期望藉此護理經驗能提供護理人員之照護參考。

This article was a nursing experience of a recurrent HCC patient with respiratory failure and pain. Care period 2014/7/2-2014/7/14. Primary care problem include: 1. Ineffective airway clearance, 2. Pain, 3. Anxiety. First, medical team discuss to design individual nursing plan to provide appropriate care measures in order to teach the patient effective deep breathing, coughing skills and weaning the respirator successfully. Second, by pain control and relaxation methods that patient can exercise pain control techniques to relieve pain. Third, empathy care to provide patient knowledge and education on the medical care to improve anxiety. Patient can obtain integrity and continuity care during hospitalization. This care experience as a reference for clinical work.
