中醫治療貝爾氏麻痺之病例報告
Treatment of Bell's Palsy by Combined Traditional Chinese and Western Medicine : A Case Report
摘要
貝爾氏麻痺是指不明原因造成的急性的周邊面神經麻痺，最明顯的症狀與徵象就是面部癱瘓，雖然不會致命，但是會對病患的身心造成巨大的影響。中醫學認為病因病機是脈絡虛空，衛外不固，外感風邪，風寒或風熱之邪乘虛侵襲阻滯面部足陽明胃經和足少陽膽經經絡，筋肉失養所致。中醫學治療貝爾氏麻痺主要可分為中藥、針刺與電針等治療方式。本案例報告將針對一位41歲女性貝爾氏麻痺病患，分析其病因病機以及運用中藥與針刺治療之成效。本案例貝爾氏麻痺病患先經過西醫類固醇藥物以及維生素B群的治療之後，再尋求中醫治療。中醫師初診評估為重度貝爾氏麻痺病患（House-Brachmann Scale: Grade V），使用中藥與針刺的中醫治療一個半月之後，幾乎完全康復。

Bell's palsy is an acute peripheral facial nerve paralysis of unknown cause. The most obvious symptoms and signs are facial paralysis, which, although not fatal, can have a huge physical and mental impact on the patient. Traditional Chinese medicine believes that the etiology and pathogenesis are caused by the emptiness of the energy, the lack of solidity of self-defense, the exogenous wind pathogens, wind-cold or wind-heat pathogens attacking and blocking the Yangming stomach meridian and the Shaoyang gallbladder meridian, causing the muscles of facial expression paralyzed. Chinese medicine treatment of Bell's palsy can be mainly divided into traditional Chinese medicine, acupuncture and electroacupuncture. This case report will analyze the etiology and pathogenesis of a 41-year-old female patient with Bell's palsy, as well as the effect of using traditional Chinese medicine and acupuncture. In this case, the patient with Bell's palsy was treated with western medicine steroid drugs and vitamin B complex before seeking Chinese medicine treatment. The TCM physician evaluated the patient as a severe Bell's palsy (House-Brachmann Scale: Grade V)on herfirst visit. After one and a half months of TCM treatment with traditional Chinese medicine and acupuncture, she recovered almost completely.
