一位乳癌病人初次接受化學治療之護理經驗
Nursing Experience of a Breast Cancer Patient Receiving Chemotherapy for the First Time
摘要
本文探討一位初次診斷罹患乳癌病人，乳房手術後於門診首次接受化學治療期間之護理經驗，筆者於2016年8月16日至2016年9月13日護理期間，運用Gordon十一項功能性健康評估工具，藉由會談、評估、觀察照護，收集主、客觀資料，確定個案在接受化學治療期間有無效性保護能力、口腔黏膜障礙、能力增進的準備度、身體心像紊亂等健康問題。筆者在個案初次接受化學治療過程中，與其建立信任感，提供週邊中心靜脈導管居家自我照護措施，同時與腫瘤照護團隊合作，提供整合性照護，並轉介木棉花病友團體分享自身乳癌治療經驗和正向心理支持，引導個案說出對身體心像改變的負向感受，陪伴其堅強渡過生命中的幽谷，能勇敢面對身體外觀之改變，度過副作用帶來的身心不適感受，成功的完成治療。期望藉由此個案報告分享，提供護理人員未來照護此類病人之經驗，進而提升護理照護品質。

This article describes the nursing experience for a breast cancer patient who received the breast surgery and first cycle of chemotherapy. The author collected subjective and objective data through interviews and observation using Gordon's functional health assessment tool in the outpatient clinic and injection room from August 16, 2016 till September 13, 2016, Forth major health problems were identified: ineffective protection, oral mucosal disorders, the ability to improve the readiness, and body image disorders. The author established trust with the patient, provided the knowledge and practice of how to care Peripherally Inserted Central venous Catheter at home, guided the patient to release the negative feelings related to body image disorders, cooperated with an oncology team to provide integrated care, introduced members of the Kapok Concern Club to share their positive cancer experiences and provide psychological support, accompanied the patient to get through the valley of life, enabled the patient to face physical changes bravely, worked together with the patient to manage physical and psychological distress related to side effects of treatment, and finally completed the treatment successfully. By sharing this case report, we provide the model of clinical advanced care of a breast cancer patient and hope the experience will help oncology nurse to improve their quality of care.
