本文為一位因面臨奶水不足導致無法如預期哺餵母乳之經產婦照護經驗,照護期間由產前2021年11月01日至11月20日、生產住院期間11月21日至11月24日、產後返家追蹤11月25日至2022年05月31日,以生理、心理、社會與靈性四大層面進行整體性評估,歸納健康問題為「低效性母乳哺餵」、「急性疼痛」、「疲憊」。筆者以個案管理師角色從個案產前檢查開始接觸,評估其需求及產後恢復情況,彈性調整訪視時間給予完整及連續性護理指導,運用母嬰親善技巧向個案衛教產後哺乳計劃,克服奶水不足問題,達成成功母乳哺餵。鼓勵家人共同參與學習新生兒照顧技巧,照護過程適時給予正向讚美、關懷、情緒支持與鼓勵,發揮母嬰親善個案管理師之護理獨特功能,協助改善產後急性疼痛與疲憊問題,期望透過分享能提供日後照護類似個案之參考。
This article is a nursing experience of a multipartum woman who was unable to breastfeed as expected due to insufficient milk supply. We collected data through total care, from November 1th, 2017 to May 31nd, 2018. Conducting a holistic assessment of the physical, psychological, society spirituality and found to have health problems including were as "ineffective breastfeeding", "acute pain", and "fatigue". As a case managers, contact the case from the prenatal examination, assess its needs and postpartum recovery, flexibly adjust the visit time to give complete and continuous nursing guidance, use baby-friendly skills to educate the case about the postpartum breastfeeding plan, and increase breastfeeding probability of success. Encourage family members to participate in the learning of newborn care skills, and give positive praise, care, emotional support and encouragement in a timely manner during the care process, and give play to the unique nursing function of baby-friendly case managers, hoping to provide reference for future care of similar cases.
