從轉譯科學觀點探究事件相關去同步(Event-related-desynchronization, ERD)應用於中風患者之復健

Applied Event-Related-Desynchronization (ERD) to Rehabilitation in Patients with Stroke: A Perspective of Translational Medicine

摘要


本篇目的以美國國家衛生研究院所提出的藍色公路模型(Blue highways)為主體,探討以腦電圖(Electroencephalography, EEG)所測得的事件相關去同步(Event-related-desynchronization, ERD)應用至中風病人之復健成效。腦電圖可以反映出數千或數萬個神經元同時活化的現象。一群活化的神經元同時減少,則稱之為事件相關去同步(ERD)。而事件相關去同步(ERD)可以在不同頻帶中觀察到,大腦會根據不同任務活化不同頻帶。因此許多研究開始應用事件相關去同步(ERD)去看中風病人之復健成效。本篇將以轉譯科學觀點探討事件相關去同步(ERD),從Bench(事件相關去同步的發現和量化)後,透過T1的小規模人體前驅試驗(建立事件相關去同步可測之面向和信度)而推演至Bedside的人體臨床試驗階段,目前的發展至T2階段,但仍需要更多的大型研究來補足,進而應用至臨床實務上。若未來能有更多研究相繼產出,可以將其應用至中風病人治療成效、預測病人預後和各受損部位中風病人所適合之治療等研究方向。藉此了解轉譯科學精神,並且提出未來可以做的研究方向,促使事件相關去同步(ERD)的基礎研究,可以快速地應用在臨床上,造福更多人類健康福祉。

This study aims to use the "Blue highways" proposed by National Institutes of Health (NIH) as the main body to investigate the rehabilitation effect of applying event-related-desynchronization (ERD) in stroke patients. Electroencephalography (EEG) can reflect the coactivation of several ten thousands of neurons. ERD is interpreted as the desynchronized activities of the activated neurons due to externally or internally events. ERD is divided into different frequency, and our brain will active specific frequency according to different tasks. Thus, recent studies apply ERD in the research of stroke rehabilitation to explore the treatment effects. This study uses translational medicine perspective to explore the process from the development, quantification and reliability establishment of ERD (basic research) to treatment effect and prognosis on stroke patients (applied research). According to this study, we can realize the core value of the translational medicine and the trends of the future research. Finally, translational research promote the basic research of ERD to apply on the clinical practice rapidly, which benefit the well-being of human beings.
