中西醫結合治療缺氧缺血性腦病變－病例報告

Hypoxic Ischemic Encephalopathy Treated with the Combination of Western and Traditional Chinese Medicine-Case Report

摘要

缺氧缺血性腦病變乃因全身性缺氧及腦血流減少，導致功能喪失；本例乃因產後大出血引發低血容性休克所致。缺氧缺血性腦病變復原之黃金時期為損傷後三個月內，這段期間大部分在住院。經由西醫會診，對缺氧缺血性腦病變住院患者進行中西醫結合治療，中醫部分以針灸治療為主，西醫部分依常規治療，本例的昏迷指數及巴氏量表由會診前的8分及0分，進步到治療後的15分及100分，療效顯著。若能建立中西醫結合治療的臨床路徑，更能掌握治療的黃金時期，進而提昇療效並縮短療程，使醫療成本降低。

The neurological deficits of hypoxic ischemia encephalopathy (HIE) are due to systemic hypoxia and decreased brain blood flow. This case was caused by hypovolemic shock following her massive post-partum hemorrhage. The golden period for recovery from HIE is 3 months after the episode which is during admission among most cases. We treat the HIE patients with the combination of Western and traditional Chinese medicine (WM & TCM) via the consultation from western medical department. The patients treated in TCM are major with acupuncture and in WM are routine. The therapeutic effect is excellent in this case, her GCS/Barthel's index was getting from 8/0 to 15/100, before and after treatment. We can promote the therapeutic effect, shorten the course of treatment, and reduce the medical cost if the clinical pathway of therapy of combining WM & TCM is set up.
