摘要
產褥熱可因於泌尿系統感染、乳腺發炎、肺炎、子宮內膜炎、傷口感染、乳腺鬱積等因素，當審因辨治以求對症下藥方能迅速見效。本案例為頭一胎生產之30歲年輕女性，從產檢到生產均在設備完善醫師陣容堅強之教學醫院進行，即便如此仍無法免於產褥熱之威脅。症見發熱、乳癰併產後惡露不絕，就診前已使用近一個月的抗生素以及退燒藥物治療仍持續反覆發燒。處方以清熱解毒的蒲公英桔梗，疏通乳腺的路路通，養血止血的阿膠，溫經暖胞的艾葉、清熱解毒升陽舉陷的升麻，行血止血去瘀生新的茜草，補肝腎益精髓的杜仲熟地菟絲子，補中益氣健脾胃的黨參茯苓白朮，活血袪瘀除癥瘕結塊的川芎，行氣降氣破滯氣的枳殼。整體而言清熱散結乳汁自通，養血暖胞則血自止惡露易停，健脾益氣則元氣提升自能禦邪，攻守得宜故能在短時間內得到良效。

Many diseases such as mastitis, endometritis, childbirth wound infection, milk stasis, urinary tract infection and pneumonia may cause puerperal fever. We must find out the underline disease in order to make an effective prescription. This 30 years old young female patient came to our OPD because the fever took place after delivery 7 days and persisted till now almost 1 month. The whole antenatal examinations and delivery process were performed in obstetrics department of medical center. Even though, she cannot free from puerperal fever. The symptoms of the patient included fever, mastitis, prolonged lochia and she also took antibiotics and NSAIDs medications for 1 month. We gave the prescriptions includes Pugongying, Jiegeng, Lulutong, Ejiao, Aiye, Shengma, Qiancao, Duzhong, Shudihuang, Tusizi, Dangshen, Fuling, Baizhu, Chuanxiong, Zhiqiao to overcome the puerperal fever and the patient rapidly got a good outcome.
