照顧一位喉癌病人接受化學治療合併使用放射線治療之護理經驗
Nursing Experience with a Patient with Laryngeal Cancer Receiving Concurrent Chemoradiotherapy
摘要
本篇主要是探討一位喉癌病人接受“化學治療合併使用放射線治療”（concomitant concurrent chemo-radiation therapy，CCRT）於治療期間所面臨的身心衝擊。筆者於94年6月19日至94年7月8日以Gordon十一項健康型態為評估架構，發現個案產生身心、社會適應困難之健康障礙問題包含疼痛、保護能力改變、口腔黏膜改變、腹瀉、身體心像紊亂。在護理過程中，筆者運用與個案建立良好的治療性人際關係，提供持續性護理指導，積極處理個案生理上的不適，並給予心理支持及鼓勵，使個案不致於中斷CCRT之療程，進而增加對疾病的自信心及適應現實的變化，能接受持續性且完整的治療，讓疾病達到有效控制。

This article discusses the mental and physical impact of concurrent chemoradiotherapy (CCRT) on a patient with laryngeal cancer. Gordon's functional health scale was used to assess the patient over 3(1/2) weeks of hospitalization. Major nursing problems were 1) pain, 2) change in protection, 3) oral mucositis, 4) diarrhea, and 5) disruption of body image. Nursing care involved establishment of a good therapeutic interpersonal relationship with the patient, provision of mental support and encouragement, and continued nursing instruction through appropriate communication and management of the symptoms associated with the disease and treatment. The purpose was to enhance the patient's self-confidence and capacity for adjustment so that the treatment program could be completed.
