喪親是椎心之痛的失落經驗,是生命過程中很難避免的經歷。本文描述一位車禍導致身體創傷同時面臨喪偶哀傷急診病人之護理經驗,護理期間從2013年4月28日至5月4日病人由急診入院、轉入病房至出院止。經由身體評估、觀察、會談、病歷收集資料,針對個案生理、心理、社會、靈性層面並依Worden哀傷反應進行整理性護理評估,發現個案有呼吸困難、疼痛及哀傷等健康問題。照護過程中與個案及家屬建立信任的護病關係,維持胸腔引流通暢、給氧以維持生命徵象與SPO2穩定;藉由放鬆技巧、音樂及藥物治療減輕其疼痛;透過醫療整合團隊共照合作,提供人性化之全人照護,藉由持續關懷、傾聽、同理並強化其家庭社會支持功能,幫助個案面對突發意外而帶來身、心創傷與衝擊,運用哀傷輔導技巧陪伴並面對喪偶之哀傷過程。藉此護理經驗之分享,期望能提供護理人員在照護類似個案之參考。
The experience of bereavement is one of the most profound grief, spiritual pain, and unavoidable life events. This paper described the nursing care experience for a middleaged man suffered from a traffic accident and spousal bereavement. The nursing care period was from April 28th to May 4th of 2013. With application of Worden's grief counseling principles, data was collected by a physical assessment, observation, interview, listening, and review of medical records to provide a framework to address the physiological, psychological, social and spiritual needs of the patient. The nursing diagnosis included dyspnea, acute pain and grief. In the nursing process, the author established a good nurse-patient relationship, maintained an unobstructed closed thoracic drainage and stable vital signs through applicable hemodynamic support. The author relieved patient's pain using relaxation skills, music and medicines. Through professional team work, the authors implemented the following nursing intervention: holistic care, constant caring with empathy, active listening and Worden's grief counseling skills to enhance external social support system, to help the patient to face physical and psychological problems from the traffic accident, and assisted the client pass quickly through his grieving. The authors share this nursing experience and hope it may be used as a reference for the nurses taking care of those patients with similar condition.
