淋巴水腫為末期病人最常見的症狀之一,正確執行淋巴水腫按摩照護能減輕不適症狀、促進舒適,臨床上發現外籍看護執行淋巴水腫按摩正確率低,易使病人生活品質降低,專案小組進行分析,發現主要問題為:缺乏外籍看護母語衛教影片、缺乏母語衛教指導單張及缺乏雙向溝通工具,並進行改善方案:1.更新淋巴水腫按摩標準手法之教育、2.新編制外語版本衛教單張及拍攝外語影片和配音、3.採用資訊化連結衛教工具網址、4.增加溝通工具等措施後,外籍看護執行淋巴水腫按摩認知之正確率由改善前平均正確率55.8%提升為91%以及外籍看護執行淋巴水腫按摩技巧之正確率由改善前平均正確率66%提升為86%達目標值。
Lymphedema is one of the most common symptoms occurred among terminal patients. Lymphatic massage can reduce swelling and improve circulation throughout the lymphatic system. Proper massages to facilitate lymphatic drainage may can alleviate discomfort and promote comfort for the critically ill.The literature shows lymphedema massages has not been performed properly by foreign caretakers, leading to a comparatively low quality of life. The purpose of this hospice care project was to increase the accuracy rate of manual lymphatic drainage massage performed by foreign caretakers. A number of barriers were identified, including: a lack of general knowledge about how to perform the massage, dearth information in their native language, and the absence of a tool suitable for cross-cultural communication. To enhance the massage accuracy rate, proposed several improvement measures were implemented, including: 1) updating and standardizing the technique for lymphedema massage, 2) preparing multimedia in bilingual languages, such as nursing care education videoclips, instructions guide and pamphlets, and 3) arranging education training for nursing staffs, and 4) amending informatic tools with website links to facilitate communications between for nurses and foreign caretakers.The project has successfully increased foreign caretakers' understandings of lymphedema massage from 55.8% to 91%. In addition, the accuracy rate of performing massage techniques also has increased from 66% to 86%, reaching the predetermined target value.
