以永續發展為導向之長期照護機構的健康促進方案:高齡者精準運動模式

Research on Sustainable Development-Oriented Health Promotion Program for Long-term Care Institutions: A Precision Exercise Intervention Model for Older Adults

摘要


本方案旨在探討以精準運動介入為基礎之健康促進方案在長照機構的應用與永續發展策略。隨著臺灣邁入超高齡社會,高齡者之健康促進日益重要。為建立系統性且個人化的健康促進模式,以WHO健康老化概念,建構以精準運動為核心之健康管理系統,其介入架構包含五大要素:(1)個案管理;(2)運動前健康篩檢;(3)運動前評估;(4)運動處方建議;(5)運動監控與反饋。在管理模式方面,整合標準化評估工具與前後測評估機制,並建立跨專業團隊合作模式,以提升方案執行效能。此方案期望藉由導入個人化的運動介入策略,強化長照機構之健康促進系統運作效能,提升高齡住民的運動參與度及身心健康狀況。此方案與聯合國永續發展目標(SDGs)相呼應,特別是在促進健康福祉(SDG 3)方面,建立可持續的健康促進模式,同時提供可及性高的運動方案,確保長者獲得平等的健康促進機會。期望為長照機構的健康促進方案提供創新介入模式與實務策略。

This content delves into the implementation of health promotion programs and the formulation of sustainable strategies through precision exercise interventions within long-term care facilities. As Taiwan enters a super-aging society, health promotion for the elderly becomes increasingly important. To establish a systematic and personalized health promotion model, this study is based on the WHO concept of healthy aging. It constructs a health management system with precision exercise as the core. Its intervention structure includes five major elements: (1) case management; (2) pre-exercise health screening; (3) pre-exercise assessment; (4) exercise prescription recommendations; and (5) exercise monitoring and feedback. In terms of management models, standardized evaluation tools, pre-and post-test evaluation mechanisms are integrated, and a cross-professional team cooperation model is established to improve program execution efficiency. This health promotion program hopes to strengthen the operational effectiveness of the health promotion system of long-term care institutions and improve the exercise participation and physical and mental health status of elderly residents by introducing personalized exercise intervention strategies. This program echoes the United Nations Sustainable Development Goals (SDGs), especially in terms of promoting health and well-being (SDG 3), establishing a sustainable health promotion model while providing highly accessible exercise programs to ensure that the elderly receive equal health promotion opportunities. This study hopes to provide innovative intervention models and practical strategies for health promotion programs in long-term care institutions.
