目的:經由翻轉教學模式及實作導向輔導方式介入對進階N3護理人員成效探討。方法:採單組時間序列之介入設計,選取某區域教學醫院N2護理人員符合可參加晉升N3條件者為對象,以翻轉式教學模式及實作導向的個案報告輔導寫作介入,評量其過程面與結果面之成效。所收集的資料以GEE(Generalized Estimating Equations)進行重複測量資料分析,以比較研究對象在課程前、中、後之差異程度。結果:共收案53名護理人員,出席率為92.8%;作業完成率95.6%;課程介入後學術能力提升0.11分(Wald X^2=1.17, p=.28),教學能力提升0.09分(Wald X^2=0.58, p=.45)、臨床實務能力提升0.12分(Wald X^2=0.94,p=.33);成就感提升0.13分(Wald X^2=0.89, p=.35)及滿意度增加0.20分(Wald X^2=4.06, p=.07),但皆未達統計上顯著差異。追蹤至2022年6月,個案報告完成29篇(54.7%)及通過審查23篇(79.3%)。結論:在個案報告完成篇數及台灣護理學會通過篇數相較於歷年佳。建議繼續沿用此模式,以追蹤其長期成效。
Objective: Explore the effectiveness of flipped learning models and practice-oriented guidance intervention on nurse clinical ladder advancement to N3. Methods: This study adopted a one-group time-series intervention design and selected, from a regional teaching hospital, N2 nurses who were eligible to participate in nurse clinical ladder advancement to N3. A flipped learning model and practice-oriented intervention or guided case report-writing was adopted to evaluate the effectiveness regarding the process and the results. The collected data were analyzed using generalized estimating equations to facilitate analysis of repeated measurement data and compare the differences of the participants before, during, and after the intervention. Results: This study recruited 53 nurses. The attendance and assignment-completion rates were 92.8% and 95.6%, respectively. After the participants underwent the course intervention, their academic performance, teaching performance, clinical practice performance, sense of achievement, and satisfaction increased by 0.11 (Wald X^2 = 1.17, p = .28), 0.09 (Wald X^2 = 0.58, p = .45), 0.12 (Wald X^2 = 0.94, p = .33), 0.13 (Wald X^2 = 0.89, p = .35), and 0.20 (Wald X^2 = 4.06, p = .07) points, respectively. However, these differences were not significant. The follow-up of the participants concluded in June 2022. A total of 29 case reports were completed (54.7%), 23 of which passed the review (79.3%). Conclusions: The number of completed case reports and the number of reports that passed the review process of the Taiwan Nurses Association were higher than those in previous years. The use of the model should be continued to enable follow-ups for assessing its long-term effects.
