有無確診COVID-19之血液透析病人日常生活健康現況與家庭照顧者負荷關聯性研究

Survey on the Correlation between the Daily Health Status of Hemodialysis Patients with and without Confirmed COVID-19 and the Workload of Family Caregivers

摘要


血液透析病人免疫力低合併慢性疾病,一旦確診COVID-19死亡率高達20%。家庭照顧者因照顧透析病人須承受身、心、社會等負荷,隨著透析病人確診COVID-19病情惡化,增加照顧負荷嚴重影響生活,形成一惡性循環。本研究目的旨在探討有無確診COVID-19血液透析病人日常生活健康現況與家庭照顧者負荷之間的關聯,並針對研究結果提出相關建議,本研究為橫斷式研究,以結構式問卷針對血液透析病人及家庭照顧者進行調查。本研究共發放102份透析病人問卷以及50份家庭照顧者問卷,以SPSS23進行統計分析,包括描述性分析、獨立樣本t檢定,以及單因子變異數分析(ANOVA),研究結果顯示血液透析病人日常生活總平均3.67,社會總平均3.6與透析病人部分特質「透析瘻管」、「運動天數」「自覺健康狀況」統計上有顯著差異,顯示COVID-19會影響透析治療病人的日常生活和健康現況。家庭照顧者在「性別」、「照顧時間」對照顧負荷統計上有顯著差異。結論:讓透析照護人員瞭解COVID-19血液透析病人生活健康現況以及家庭照顧者的照顧負荷關聯,以提升更優質的照護環境與生活品質。

Hemodialysis patients have compromised immune systems and often suffer from chronic diseases, leading to a COVID-19 mortality rate of up to 20% once diagnosed. Family caregivers bear significant physical, psychological, and social burdens when caring for hemodialysis patients. The deterioration of the patient's condition due to COVID-19 exacerbates the caregiver's burden, severely affecting their lives and creating a vicious cycle. This study aims to explore the relationship between the daily health status of hemodialysis patients with or without COVID-19 diagnosis and the burden on family caregivers. Based on the study results, relevant recommendations will be made. This cross-sectional study utilized structured questionnaires to survey hemodialysis patients and their family caregivers. A total of 102 questionnaires were distributed to hemodialysis patients and 50 to family caregivers. Statistical analysis was performed using SPSS 23, including descriptive analysis, independent sample t-tests, and one-way ANOVA. The study results showed that the overall average daily life score for hemodialysis patients was 3.67, and the overall social average was 3.6. Significant differences were found in certain patient characteristics such as "dialysis fistula," "days of exercise," and "perceived health status," indicating that COVID-19 impacts the daily life and health status of dialysis patients. Among family caregivers, significant differences in caregiver burden were observed based on "gender" and "caregiving duration." Understanding the relationship between the daily health status of COVID-19 diagnosed hemodialysis patients and the burden on family caregivers can help dialysis care providers enhance the quality of care environments and improve the quality of life for both patients and caregivers.
