Acute appendicitis is the most common surgical problem in the abdomen. It usually presents as right lower abdominal pain and rarely, as left lower abdominal pain clinically. We report a 30-year-old woman with left appendicitis due to situs inversus totalis. She received appendectomy and recovered smoothly. We want to remind emergency physicians to keep this condition in mind.
